The Director of Community Engagement’s primary responsibility is to design, build, manage and scale the company’s outreach to community-based organizations related to social determinants of health. The Director of Community Engagement will collaborate with Implementations, Product, Customer Success, and Engineering and reports to the VP, Operations.

 

You will:

  • Serve as the leader and manager of the Community Connect team, driving strategy and execution of all things related to recruitment, onboarding, and engagement of community-based organizations related to social determinants of health.
  • Drive awareness, engagement, and engagement of community-based organizations to ensure sustained growth and vibrancy of the community, reaching new audiences and bridging existing ones.
  • Own key engagement metrics and the overall success of engagement initiatives.
  • Plan, execute, and host physical and virtual events, user groups, and meetups to connect community members to company products and services.
  • Expand the content footprint of the Community Connect team through various mediums and channels: video, audio, social, external communities, conferences, other offline events.
  • Define job descriptions, best practices, and performance metrics for the Community Connect team, performing supervision and basic human resource tasks such as hiring, training, mentoring, and conducting performance evaluations.

 

You have:

  • A track record of leading cross-functional initiatives, integrating technology, operations, marketing, and communications activities toward achieving strategic business outcomes.
  • Hands-on experience with nonprofits, community-based organizations, and community initiatives and an ability to connect to both leadership and operational staff.
  • An understanding of how social determinants connect to healthcare outcomes and how existing healthcare delivery systems might partner with community-based resources to address them.
  • A warm and welcoming customer-facing personality.
  • Excellent written and oral communication.

ACT.md seeks experienced sales executives to build markets and drive sales in a complex healthcare environment.

 

ACT.md is a Boston-based healthcare technology startup working to disrupt healthcare’s current fragmented approach to patient care, care coordination, and helping address patients’ social determinants that keep them from achieving their best health.

 

The company was founded in 2012 to develop better, safer, cheaper tools for managing healthcare’s most complex patients.  ACT.md allows providers, social workers, patients, families, or any person helping to provide care for a patient to access and contribute to a task-based project plan for patients.

 

ACT.md clients include a range of academic medical center, payers, and government and community agencies that are involved in the overall care of a patient population. The main focus of the position is to build the ACT.md customer base, both through direct selling and through support of channel partners.  

 

This individual will rapidly develop an intimate understanding of a wide array of prospect situations and needs, interpret those needs for appropriate positioning of ACT.md's technical and service offering, and reliably drive process to closure. To be successful, this individual must be quickly viewed by prospects as a credible advisor to their strategic efforts to be successful under new care delivery models and manage complex care across the continuum. They must have a fundamental understanding of the care delivery process and reimbursement models and be able to communicate with a variety of constituents including clinical, technical, and operational leaders.

 

This person needs to have an entrepreneurial spirit and strong organization, communication and leadership skills. The Sales Director must have a high sense of integrity and personal responsibility and align with ACT.md values of passion and excellence. Teamness, flexibility and aptitude for learning are critical traits.

 

Essential Duties and Responsibilities

  • Effectively communicate ACT.md value proposition, differentiators, and messaging
  • Own market segment: develop and execute strategy to build pipeline and drive business forward
  • Manage funnel (forecast accuracy and activity prioritization)

Skills & Knowledge

  • Well versed in healthcare payment policies and payment reform trends
  • RN, BSN, MSN, or MPH degree
  • 5+ years of healthcare consulting or sales experience
  • Knowledge of and experience consulting organizations pursuing team-based care models and performance based contracts is preferred
  • Experience selling to both provider and payer organizations preferred
  • Previous experience working as part of a growing early stage company preferred

Andrea is a care coordinator helping navigate hundreds of patients through complex care. Each day, she needs to focus on the most critical patients before their care veers off track, and guide their care teams back to an ideal trajectory. Using ACT.md’s web-based platform, Andrea can launch a targeted series of care workflows to ensure each patient gets the best possible treatment, especially when care crosses organizational boundaries, includes family caregivers, or requires the engagement of community services. In concert, ACT.md integrates each patient’s care plan with data from hospitals, health information exchanges, or insurers, so that automated or human decision-making is always well-informed.

 

You will:

  • Take a deep dive into our codebase, building new features and improving our existing functionality.
  • Actively pair with colleagues through all stages of our process: architecture, planning, development, and code review.
  • Work closely with our UX and Account Services teams to understand our users—patients, families, clinicians, case managers, and community resources—so we always build functionality that matters.
  • Engage fellow team members and users to develop and refine design concepts for our web and mobile applications
  • Guarantee code quality and user experience through automated testing, ranging from isolated unit tests to in-browser integration tests.

We seek an experienced leader to fill a senior Implementations and Account Services role. 

 

ACT.md’s cloud-based CareHub™ connects patients, families, care teams, and community services together on the journey to health and well-being. We are looking for a creative problem-solver who combines visionary leadership with hands-on implementation. From sales through implementation and account growth, this individual will need to build collaborative client relationships, and support change management across a variety of settings, including clinics, hospitals, and non-traditional healthcare settings.

 

You will:


- Analyze workflows of clinical programs and processes and implement a customized solution that combines technology and service lines to meet client needs
- Develop programs and manage projects from start to finish, effectively
Identify new business development opportunities and spearhead expansion projects within accounts
- Create and deliver implementation training programs, including producing print, digital/interactive training materials

 

You are:


- An advanced-degree professional with at least 5 years professional experience in a health or social services related field AND in either an account services, program management, or consulting role
- Experienced and comfortable presenting to individuals and groups at all organizational levels including executives and prospects
- Experienced mentoring on-site and remote staff
- Tech savvy, and preferably a data wiz!
- Experienced with change management process, and sensitive to workflow disruption at clinical sites

The Technical Project Manager (TPM) works with internal business owners, current and prospective clients, and channel partners to deliver web-based software engineering projects.

 

The TPM leads and manages the process of translating stakeholder needs and requirements into scoping and specification materials, manages the human resources required to execute the projects, sets and maintains project plans and milestones, and assists in the creation of documentation and release materials around the projects they complete. For client-facing projects, the TPM is involved from early in the pre-sales cycle (RFPs, proposals) through implementation and client sign-off. The TPM must be able to manage many small projects at a fast pace, balancing speed of execution with the need for documentation and quality and must have a full understanding of software development methodologies, best practices, and common web application architectures.

 

The Technical Project Manager role requires a strong web-based technical background, excellent client and project management skills, and the ability to lead technical teams that include internal and client resources. Successful candidates must be action-oriented, capable of independently driving solutions to technical problems, and able to communicate clearly and effectively to both technical and business stakeholders.
